5099, 6000, 6310, 7000, 8000 Course Request Form

Please submit this form at least one day before classes begin. Requests submitted after classes start cannot be processed and must be handled by the Registrar.

Before filling out this form, please contact the professor and obtain permission. Also check the current schedules first to see if there is already a course open: Current schedules (opens in a new tab) .

You can register the requested class after it appears in the current schedules page. Note that all the 5099, 6000, 6310, 7000, 8000 courses are variable credit courses.
