TexPREP - Lubbock Application Checklist

1) Receive parent's or guardian's permission.

2) Read the entire application.

3) Answer all questions on the application.

4) Complete the essay.

5) Include a recent grade report.

Give the two nomination forms (included with the application) to your teachers or counselor to complete and submit. One must be from your Math teacher and the other must be from your English or Science teacher or your counselor. Please have your teachers send nominations in school envelopes directly to TexPREP.

Make a copy of the application for your records.

Note that a completed application includes a copy of your latest report card, a completed application form, two nomination forms from teachers or counselor (sent directly from them), and an essay.

A student's application will not be considered for acceptance without the completion and submission of all of the above items.
